Donna Phillips, PhD, PT, CHT
Chief Executive Officer
Dr. Phillips brings more than 30 years of physical therapy expertise to HealthSouth Rehabilitation Hospital of Fredericksburg. She graduated from Rutgers with a bachelor’s degree in physical education and received a master’s degree in physical education from Temple University in Philadelphia. She received a post-baccalaureate certificate in physical therapy from Hahnemann College and her doctorate in pathokinesiology, the study of abnormal movement, from New York University. She has been a clinical associate professor of physical therapy at the University of Medicine and Dentistry of New Jersey since 1995.
Joseph Paquette, M.D.
Medical Director
Dr. Paquette specializes in family practice and is the medical director at HealthSouth Rehabilitation Hospital of Fredericksburg. He also serves as chairman of HealthSouth’s medical executive committee and president and chief medical officer at Pratt Medical Center. A Virginia native and graduate of the Medical College of Virginia, Dr. Paquette completed an internship and a residency at the Riverside Family Practice Program. He is board certified in family medicine.
Joseph Paquette, M.D. is an independent private practice physician whose office is located at the hospital.
Donna Fleming, MSN, RN-BC, NE-BC
Chief Nursing Officer
Donna is the chief nursing officer at HealthSouth Rehabilitation Hospital of Fredericksburg and has extensive acute care and leadership experience. She received her associate degree in nursing from Germanna Community College, her bachelor’s degree from Virginia Commonwealth University and a master’s degree in nursing administration from George Mason University. Donna also holds a specialty certification from the American Nurses Credentialing Center (ANCC) in Medical-Surgical Nursing and as a Nurse Executive.
Yolanda Johnson
Human Resources Director
Yolanda is the human resources director for HealthSouth Rehabilitation Hospital of Fredericksburg. She is a South Carolina native and brings with her extensive human resources experience. Prior to joining HealthSouth, Yolanda served as an HR consultant for UVA Transitional Care Hospital and talent acquisition manager for the UVA Health System. She is a graduate of Columbia College where she earned a bachelor’s degree in business, and a graduate of Pennsylvania State University earning a master’s degree in human resources and employee relations.
Michael Jones
Director of Plant Operations
Michael is the director of plant operations for HealthSouth Rehabilitation Hospital of Fredericksburg. A retired Navy Chief Petty Officer, he brings with him more than 28 years experience in plant maintenance. Michael began his Navy service aboard the USS Aylwin in Charleston, South Carolina. He then led the air conditioning, refrigeration and air compressor shop at the Shore Intermediate Maintenance Activity in Rhode Island. Michael was stationed in Sabana Seca, Puerto Rico, where he served as Patrol Supervisor and Physical Security Officer. He was also stationed aboard the USS Seattle in Earle, New Jersey, and was later assigned to Naval Healthcare New England in Rhode Island. He finished his naval career as manager of a satellite clinic serving officer indoctrination candidates.
Phu Le, PT, DPT
Director of Therapy Operations
Phu is the director of therapy operations for HealthSouth Rehabilitation Hospital of Fredericksburg. He started his career at Mary Washington Hospital with experience in inpatient rehabilitation, outpatient orthopedics, outpatient neurological and day rehabilitation. Phu graduated from University of St. Augustine in 2001 with a master’s degree in physical therapy. He completed his doctorate degree in physical therapy in 2007 from Shenandoah University. Phu has lived and worked in the Fredericksburg area for more than 10 years.
Abby Lloyd
Controller
Abby began her career at HealthSouth Rehabilitation Hospital of Fredericksburg in August 2007. She started in the accounting department as the accounts payable/payroll specialist, working in that role for more than four years. She was promoted to the controller position in 2012. Abby holds a bachelor of professional studies degree from the University of Mary Washington.
Lisa Mundy
Director of Marketing Operations
Lisa is the director of marketing for HealthSouth Rehabilitation Hospital of Fredericksburg and brings with her more than 20 years of experience in medical sales, marketing, and physician relations. Prior to joining HealthSouth, she was the admissions director at a skilled nursing facility. Lisa is a Maryland native who graduated with a bachelor’s degree in animal science from Virginia Tech.
Rebekah Price, RN
Director of Quality and Risk Management
Rebekah has been employed by HealthSouth Rehabilitation Hospital of Fredericksburg since 2009, bringing more than 30 years of acute care nursing experience to her position. She completed her nursing studies at Alexandria Hospital School of Nursing, and holds a bachelor’s degree in forensic behavioral science from Nova Southeastern University. She is a member of Association for Professionals in Infection Control and Epidemiology, and is currently working on her certification in infection prevention.
Karen Truslow, RN, MSN, ACM, CCM
Director of Care Management
Karen brings 18 years of case management experience to HealthSouth Rehabilitation Hospital of Fredericksburg. Prior to joining HealthSouth, she served as a case manager/operations manager for the department of care management at Mary Washington Hospital. She began her career at Mary Washington Hospital as a home health nurse. Karen received a bachelor's degree in nursing and a master's degree in nursing administration both from George Mason University. She is an Accredited Case Manager and Board Certified Case Manager.
Ravi Wankhede
Pharmacy Director
Ravi started working for HealthSouth Rehabilitation Hospital of Fredericksburg in February 2009. Prior to coming to HealthSouth, he was a pharmacist with Wal-Mart. A native of Nagpur, India, he has a bachelor’s degree in pharmacy from India. He received a master’s degree in biological science and in adult and technical education from Marshall University in Huntington, West Virginia. Ravi is also a Certified Geriatric Pharmacist.
